An amazing job opportunity has arisen for a motivated Integrated Discharge Practitioner Nurse to  work with one of UK's leading health care providers. As an Integrated Discharge Practitioner Nurse you will be working in a highly reputed hospital service within the Gillingham area.

This special hospital provides surgical and medical specialties, including the area vascular (vein) service, haematology (blood), respiratory, diabetes and heart conditions. We also have a children's service with an expanding surgical unit. Our clinical services are supported by an intensive care unit, acute pain team and infection control team.

As an Integrated Discharge Practitioner Nurse your day to day duties include:

·         Supporting ward teams with timely and safe patient discharge as well as assessing patients within the emergency department (ED)

·         Assessment areas and patient’s own home focusing on admission avoidance and encouraging independence.

·         To advise, clinically triage, assess and case manage as necessary the patient’s discharge pathway,

·         Offering alternative solutions to prevent admission or facilitating the patient’s timely discharge from the hospital.

An ideal Integrated Discharge Practitioner Nurse must be able to have these skills and meet the following requirements:

·         Put your NMC/ 1st level registration to great use

·         You will have the ability to work both independently and also as part of a wider multidisciplinary team

·         It is expected that you should have strong communication and organisation skills

·         Flexible approach which will ensure you deliver positive experiences to patients at every stage of their healthcare journey.

·         We have built organisation-wide committees in many specialist fields.

·         You’ll have the option to join your clinical specialism, networking with professionals from across the country.

·         We get together regularly, auditing and sharing standard methodology for innovation and service re-design.

·         You’ll be a part of the pool of subject matter experts bringing together clinical and operational leads.
