An amazing job opportunity has arisen for a committed Advanced Nurse Practitioner to work with one of UK's leading health care providers. As a Advanced Nurse Practitioner you will be working in a highly reputed service within the Ormskirk area.

You will be working within the new Short Intensive Support Service team, you will provide an expert role model for the delivery of clinical and professional practice. You will provide clinical leadership for the other members of the SISS team and support with line management. Working with the clinical lead, you will deliver robust clinical governance, clinical pathways and robust data to deliver the fulfilment of the service contract.

As an Advanced Nurse Practitioner Nurse your day to day duties include:

·         You will manage a clinical case load of undifferentiated cases using telephone triage and home visits.

·         You will implement health promotion and preventative strategies and ensure communication and collaboration with referrers and patients key clinicians in primary and community services.

·         You will use enhanced skills and knowledge to assess the physical and psychosocial needs and develop a comprehensive and concordant plan of care with the patient, their carers’, and the wider MDT as needed.

An ideal Advance Nurse Practitioner must have these skills and be able to meet these requirements as they are beneficial for this position:

·         registered nurse with current registration with a clinical body,

·         Have a nurse practitioner qualification or equivalent experience

·         Advanced clinical examination skills at level 7.

·         Experience working autonomously in UCC/pre-hospital/community setting and have evidence of up to date personal and professional development.
